Accuracy of leukocyte esterase reagent strip test and platelet indices for early diagnosis of ascitic fluid infection in liver cirrhosis

Background: Spontaneous bacterial peritonitis (SBP) is one of the serious complexities in patients with cirrhosis and ascites. 30-50% death may occur if it not treated properly. The leucocyte esterase reagent (LER) strip test a special apparatus for prior detection neutrophils ascetic fluid. aim study was to investigate diagnostic accuracy LER platelet indices early SBP cirrhotic patients. Methods: This cross-sectional carried out department gastrointestinal hepatobiliary pancreatic disorders (GHPD), BIRDEM, Dhaka, Bangladesh, from January 2018 October 2019. A total 110 indoor liver ascites were enrolled after fulfilment inclusion exclusion criteria adequate history taking clinical examination. Results: Out cases, 68 male 42 female, among them 23 (20.9%) had SBP. at grade 4 set as cut-off value which showed sensitivity, specificity, positive predictive (PPV), negative (NPV), 91.3%, 90.8%, 70%, 97.5%, 90% respectively. Mean volume (MPV) distribution width (PDW) done by complete blood count, where MPV 12.5 fl PPV, NPV, 82.6%, 66%, 96.25%, 88.18% PDW on 18.10% revealed 83.9%, 50%, 95.71% 79% ROC curve plotted examining sensitivity specificity.
